Breathwork Harmonics Corp. prioritizes the safety and well-being of all our participants, and as part of our commitment to ensuring a secure environment, we require the completion of this Liability Waiver Form.
A breathing session may not be suitable for you if you have the following conditions:
· Cardiovascular problems, abnormally high or low blood pressure, aneurysms
· Diabetes
· Epilepsy and seizures in the past
· Osteoporosis
· Recent surgery
· Glaucoma
· Currently pregnant
· Anyone taking heavy medication, severe psychiatric symptoms, especially psychosis or paranoia, bipolar. And/or anyone experiencing an emotional or spiritual crisis or any person with a mental illness who is not in treatment or lacks adequate support.
· People with asthma should bring their own inhaler and consult with their physician and breathing session instructor before participating.
Please note that this list is not exhaustive. We generally advise that if you have a question regarding a condition that you may have which is not listed here, you consult a physician before participating in these breathing sessions.
I warrant and represent that I am in good health physically, mentally, psychologically and emotionally, and I understand and warrant that if I am not in good health, I will not be allowed to perform the activities and sessions. Accordingly, the declaration and certification that I am in good health in all the above-mentioned respects constitutes a material agreement to allow me to participate in the breathing sessions.
I know and acknowledge that the person facilitating is not a doctor or psychiatrist, or a specialist in health care, and that the activities offered are not intended to treat and diagnose specific medical conditions, whether physical, psychological or emotional.
I voluntarily participate in these activities knowing the risks and consequences and agree to assume all consequences, known or not. I agree to accept financial responsibility for costs related to treatment.
I release trainer Stephanie Carrothers, Owner of Breathwork Harmonics Corp. from all responsibilities, costs and damages that may arise from participating in the above-mentioned activity.
I hereby grant Breathwork Harmonics Corp permission to use photographs and/or videos of me taken during a 9D Breathwork Classes or Workshops for use in promotional materials, including but not limited to social media, website content, print materials, and advertisements.
I understand that these photographs and/or videos may be edited for aesthetic or promotional purposes, and I waive any right to inspect or approve the finished product.
I agree that all rights to the photographs and/or videos, including the right to reproduce, distribute, and display them, shall belong to Breathwork Harmonics Corp.
Cancellation Policy:
Clients and Members are encouraged to book 9D Breathwork Journeys in advance as sessions do fill up quickly.
To avoid a $40 late cancellation/no show fee, the member must cancel a class reservation within 24 hours prior to the session start time. This applies to all sessions regardless of site, including in person, studio, private residence, business, or online 9D Breathwork session.
No Show Policy:
If you schedule a session and fail to attend without prior cancellation, you will be required to book a future session through the staff of Breathwork Harmonics Corp., either by phone or email, instead of booking online. After two (2) instances of failing to attend scheduled sessions without prior notice, Breathwork Harmonics Corp. reserves the right to terminate your membership without a refund or terminate client relationship.
Drop-in/Walk-in Policy:
Drop-in/Walk-in are welcome if space permits. However, to minimize disruption to other clients, no admittance will be allowed within 15 minutes of the session start time.
Refund Policy for Class Cancellation (Non-Member):
· If you need to cancel a class, please provide us with at least 72 hours’ notice to be eligible for a refund. Cancellations made less than 72 hours before the scheduled class time are not eligible for a refund.
· To cancel and request a refund, please contact us directly through email at [email protected] with “class cancellation” in the subject line. Refunds will be processed using the original payment method within 7 – 10 business days.
